For WP Media Uploader to work with the W3 Total Cache plugin, one of the W3 Total Cache plugin Database Cache settings must be enabled:

Don't cache queries for logged in users

 must be checked

DONOTCACHEDB

 must be in the "Reject constants:" list

These are default settings for the W3 Total Cache plugin.
